Dear Intensive English Student,

 

Thank you for selecting the ELI at UTA as your place of study.  We are looking forward to working with you to improve your English language skills.  Please read this letter carefully, as it contains important information for the upcoming semester.

 

1.       Registration Information   Please review the enclosed First Week Schedule on page 2.

 

2.       Student Health Insurance Requirement The cost of the UTA health insurance is $438 per student

and will be included in your tuition payments.  Enrollment in the University health insurance

plan is required for all international students at UTA.

The only exceptions are: 

(1) Student is on a government-sponsored health plan; or

(2) Student is covered by an insurance plan from an employer.

If you will be covered by one of these categories, you must provide proof of such coverage which indicates that the policy meets the UTA and State regulations (less than $300 deductible and a minimum $100,000 benefit).

Please note that if you are not covered by one of those above categories, you must buy student health insurance from ELI/UTA.   Insurance from your home country will not be accepted.

 

3.  TB Test Requirement  All non-US citizen/non-permanent resident students are required to show proof of TB test and /or chest X-ray done in the US within the last 12 months to clear the Health ServicesŐ requirement.

Do not take the TB test before arriving in the US.  The TB test and /or chest X-ray test must be completed by UTA Health Services.

 

4.  Payment Policy Students may pay the tuition/insurance fees on August 28 or August 29, 2008.

(1) Student MUST pay all tuition and insurance fees in one full payment.

(2) The deadline for the tuition and insurance payment is Friday, August 29, 2008.

(3)    Students who do not pay all tuition and insurance payment by the deadline will be dismissed from the program soon after the deadline.

    

5.  Late Arrival Policy   Late arrival for Fall 2008 semester is not available.

            Students must be at the ELI on August 25, 2008.

 

6.  Parking   Any student who lives in off-campus housing is eligible to receive one parking decal from the ELI without charge.  During the first week of the semester the student may pay to park in the parking garage until he/she receives the parking decal.  The cost is $2.20 for the first hour, $1.65 for each additional hour and $7.15 after the 4th hour.
